• Treat all, irrespective of CD4 count and clinical stage. Reduced severe events/death (START trial, 2015). Treatment as prevention, reduces infection in sero-discordant couples (Cohen NEJM, 2011, Rodger JAMA, 2016). Undetectable=Untransmissable
